The work is relationship based, highly independent, and constantly changing. It requires more than clinical knowledge.
It requires curiosity, organization, judgment, follow through, and the ability to see how many parts of a person’s life affect one another.
You have time to understand the person, not simply complete the task.
You manage your work in the community and are trusted to use sound judgment.
You are supported by Senior Care Managers, leadership, and colleagues with different areas of expertise.
You evaluate competing needs, family dynamics, risks, and options when the answer is not obvious.

No two clients or days are exactly alike.
CMN offers more flexibility than many traditional healthcare environments, but flexibility does not mean working without structure.

Team members manage much of their own calendars and work independently in the community. In return, they are expected to take ownership, stay organized, and communicate clearly.
We thoughtfully build schedules based on client needs, employee availability, experience, and long term fit.
This work may not be the right fit for someone who prefers a highly predictable routine, needs frequent step by step direction, or wants to focus only on clinical tasks.
New Care Managers receive structured training, observation, guided practice, and ongoing feedback before managing work independently.
As skills grow, team members take on greater responsibility for client planning, coordination, communication, and decision support.
Experienced Care Managers may also have opportunities to mentor others, contribute to team development, and grow into leadership responsibilities.
We hire carefully because the right fit matters to our clients, our team, and the person stepping into the role.
We are interested in experienced registered nurses and other qualified aging professionals seeking either full time or part time work in community based care management.
Our current team needs lean toward full time availability, but part time Care Managers remain an important part of the team we are building. We welcome early conversations with professionals seeking either path so we can take the time to determine whether there is a strong mutual fit.
Companions help older adults remain engaged, connected, and supported through companionship, transportation, organization, errands, and practical assistance. We are looking for compassionate, dependable people who value relationships and take pride in making daily life easier and more enjoyable.
